Będąc in ea statione et gradu w Rzeczypospolitej […] nie zwykłem […] [wzmianka o prawach i przywilejach miasta oraz kupcach]. […] Proszę […] deklaracyi WMM […] [dalszy tekst częściowo nieczytelny].
[Datowanie:] W Łukach die 7 Augusti 1645.
